The content that is allegedly infringing and its location on the website should be identified (please provide URLs or other relevant facts).
Your address, phone number, and email address are among your contact details.
a declaration expressing your sincere belief that the use of the content is prohibited by the law, the copyright owner, or its agent.
a declaration that you are authorized to act on behalf of the copyright owner and that the information in the notification is true, under penalty of perjury.

Reverse Notification
You can submit a counter-notification to our Copyright Agent if you think that content that was blocked or deleted as a result of a DMCA notice was done so in error. This includes:
Your signature, either digital or tangible.
identifying the deleted item and determining where it was before removal.
Under penalty of perjury, you must say that you believe in good faith that the information was deleted because of an identifying error or mistake.
Your name, address, phone number, and a declaration acknowledging that the federal court in your district has jurisdiction over you.
Repeat offenders
Users who repeatedly violate the law may have their accounts terminated by Lateh.
